Overview

The Adult Outpatient Program provides an individualized approach to meet a person’s care needs and works together with the client to develop a foundation of skills one can utilize to support long-term success and wellness.

  • Provide outpatient treatment in a variety of sites to meet the needs of our clients
  • Assist individuals in increasing functioning and improving the quality of their life through stress and symptom management, development of coping skills and processing of emotions
  • Develop individualized plans of care to meet specific needs including treatment for multiple diagnoses or co-occurring substance use issues

Based on individual and clinical needs, individuals are assigned to a therapist that will best match the needs of the individual. Our staff offers specialized evidenced-based treatment in areas such as: Cognitive Behavioral Therapy, Solution Focused Therapy, Dialectical Behavioral Therapy, Trauma Focused Treatment, and Brief Therapy.

Group Therapy

Group therapy can be an ideal choice for addressing concerns and making positive changes in ones life. Groups are designed to target a specific problem, such as depression, panic disorder, social anxiety, chronic pain or substance use. Other groups focus more generally on improving social skills, helping people deal with a range of issues such as anger, shyness, loneliness and low self-esteem.

DBT Skills Group

DBT is a comprehensive cognitive-behavioral treatment that was originally developed to treat chronically suicidal individuals suffering from borderline personality disorder (BPD) or individuals who may be struggling with issues related to emotion dysregulation and quality of life instability. The treatment includes a weekly skills group, weekly individual counseling, and phone coaching. Participants are asked to sign a contract agreeing to stay in the treatment program for a select amount of time to be determined between the individual and their clinician.

Psychiatric Services

The Adult Outpatient Program offers psychiatric services, which include psychiatric assessment, medication prescribing, review and follow along, for individuals that are consistently engaged in therapeutic services in the program. Our psychiatrist also utilizes a consultation model in which our psychiatrist provides an evaluation, will stabilize the individuals medications, and refer the individual back to their primary care provider for follow-along care.

Older Vermonter Services

Clara Martin Center is part of the Tri Care Area that Washington County Mental Health serves, along with Lamoille County Mental Health, for Older Vermonter Services. The program provides in-home counseling services and referrals to home bound elderly individuals living in the community.

Modified DBT Skills Group

Modified DBT Skills Group – This group teaches the 4 core skills from DBT (mindfulness, distress tolerance, interpersonal effectiveness, and emotion regulation) with some variations from the full DBT program. Please call for details.

Case Management

Short-term case management can be available to adults, 18 years or older, who are either already engaged in outpatient services or those in the process of connecting with outpatient services.  Services are focused on supporting clients to address immediate needs that might be causing a barrier to treatment.

Community Collaborations

The Vermont Blueprint for Health

A state-led, nationally-recognized initiative that helps health care providers meet the medical and social needs of people in their communities. All Blueprint work is closely integrated with health and human service organizations through Community Collaboratives that guide care delivery and payment reforms at the local level. The Blueprint’s aim is constant: better care, better health, and better control over healthcare costs.

Support and Services At Home (SASH)

SASH is a Blueprint for Health initiative targeted at helping elderly/disabled people stay in their homes longer at optimal wellness. The program is staffed by SASH Coordinators and wellness nurses in many locations throughout the State.

Community Support Program (CSP)

The Community Support Program (CSP) assists individuals with mental health issues in achieving and sustaining the highest quality of life consistent with their abilities, needs, personal ambitions, and available resources. The program strives to instill wellness to individuals living in the community. 

Primary Health Care Integration Services

Clara Martin Center has worked to develop strong, collaborative relationships with primary care practices within our service area. Clara Martin Center is co-located with Gifford Health Center in Chelsea at the Chelsea Health Center and works closely with their offices in Randolph, Bethel and Rochester. 

Safe Haven Shelter

Safe Haven is a 4-bed transitional living shelter for people who are homeless and have a mental illness, primarily serving residents of Orange County, Washington County, and the upper towns of Windsor County. Safe Haven provides a place to live within the community while working on personal goals in a safe and supportive recovery-oriented environment.

Since its inception in 1998, a focus of the Safe Haven project has been to build a community of people with lived experience, to provide supports for guests both within the house, as well as within the larger community.
